Keep note history to prevent hindsight bias

When refining notes, it is useful to keep some sort of history. Either through version control, which can get tricky when splitting notes, or through appending updated insights with datetime stamps.

This can help you track how your thinking and mental models changed over time.

More importantly, since human memory is not trustworthy, it will prevent hindsight bias.
